SJICF Grant Application Procedures
Please complete
the Grant Application Cover Sheet, narrative sections of the grant
application, and provide the requested attachments by June 30, 2008.
Confine
the narrative section to a maximum of 5 pages, not including financial
information, 990 (first 5 pages only), and 501 (c) (3) IRS
determination letter. PLEASE SUBMIT SIX (6) COPIES OF YOUR GRANT
APPLICATION AND PROJECT BUDGET. ONLY ONE COPY (1) OF YOUR 990, IRS
DETERMINATION LETTER, PROFIT AND LOSS/ BALANCE SHEET STATEMENTS, AND
SUPPORTING DOCUMENTION IS NECESSARY.

III. Describe your Organization
(narrative)
- Briefly
describe your organization's history and major accomplishments.
- Describe your
current programs and activities.
IV. Describe the Program/Project Request
(narrative)
- What is the
scope of the project; people served or breadth of the influence of the
project?
- What are the
goals, objectives and activities and/or strategies involved in this
request?
- What
problems, needs or issues does this project address?
- What process
will you use to evaluate the project’s success or outcome; describe
benchmarks/criteria.
- What is your
timeline?
- Who will be
involved in carrying out the plans outlined in this request?
- List other
funding sources and amounts received, committed, or requested for this
project.
V. Financial Documentation (attachments)
- Your
organization's current annual operating budget, profit and loss and
balance sheet statement, and the budget for the Program/Project for
which the request is made.
- A copy of
your most current 990 (first 5 pages only) or indicate that the most
current copy is posted on www.guidestar.org.
- Attach a copy
of your IRS 501© (3) determination letter or explain your status.
